Exemptions and Waivers
Medical and Religious Exemption Requests
Students attending UMass Boston may request an exemption from any immunization requirement, subject to religious and disability accommodations pursuant to state and federal law. Note than an exemption cannot be submitted for the Tuberculosis (TB) Screening Questionnaire, as all students must complete this web-based questionnaire.
Medical Exemptions
- Download: Download the Medical Immunization Exemption Form.
- Complete: Have your healthcare provider complete and sign the form.
- Upload: For instructions on uploading records, click here. If you have immunization records, please also submit these for review.
- Review Period: Allow time for a nurse to review your submission. If approved, your immunization status will be updated to reflect your medical exemption.
Religious Exemptions
- Schedule an Appointment: To request a religious exemption, please call 617-287-5661 to schedule an appointment with a UHS Nurse.
- Please bring to your appointment a signed written statement that the vaccine(s) you are declining (specify which vaccines) conflict with your sincerely held religious beliefs.
- The statement must be signed by a parent/guardian if the individual seeking an exemption is a minor.

Meningococcal Vaccination Waiver
- Schedule an Appointment: To request a MCV4 waiver, please call 617-287-5661 to schedule an appointment with a UHS Nurse.
